Your User Name is set up the following way:

You will enter:
The first 4 letters of your last name, followed by...
The first 2 letters of your first name, followed by...
The last 4 digits of your phone number.

FOR EXAMPLE: If your name was Jacob Jones and your phone
number was 555-1234, then you would enter: joneja1234

(remember... you may have given us more than one number so be sure to try all that apply)
If your last name has less than 4 characters the we will simply use your entire last name.

Your User Password is set up as follows:
Your birthday will be used as a 6 digit number for your password.
So if your birthday was May 16, 1985 then you would enter 051685.
6 Numbers Only. No Slashes. No dashes. Just 6 Numbers that make up your birthday.
